The report (Notification) can be submitted by any Certificate Holder, Service Provider, including the entire aviation community is encouraged to file a hazard report valid within the working environment. This includes but not limited to the following items:
 

• Anything with the potential to cause harm within the working environment.
• Any condition, event, or circumstance which could induce a potential accident.
•  Any existing or potential condition that can lead to injury, illness, or death to people; damage to or loss of a system, equipment, or property; or damage to the environment.
•  A condition that might cause (is a prerequisite to) an accident or incident.
